Our Kane Community Hospital Home Health Care team provides for your health requirements with friendly, compassionate, and individualized care in the comfort of your home, working directly with you and your doctor.
KCH Home Health Care provides quality home health you can rely on -- with a nurse on call seven days a week, 24 hours a day. The KCH office moved to 628 N. Fraley Street, Kane, across Route 6 from the hospital in early 2012.
KCH Home Health Care is the only home health care/visiting nurse program associated with KCH. KCH works with UPMC Hamot-affiliated home care agencies to provide additional services such as diabetic education with the Diabetes Institute at Great Lakes Home Healthcare Services.
KCH Home Health Care is certified and licensed by the PA Department of Health as a home health agency and certified by Medicare and Medicaid. Our agency consists of local health care professionals who will come to your home to provide the services you need.
Among the services provided are skilled nursing care, diabetes, nutrition and wellness counseling, pediatric nursing, wound and ostomy care, IV therapy, tube feedings, catheters, continence care, rehabilitation services (physical, speech, and occupational therapies), home health/nurse aide, social services, and telehealth.
Telehealth, one of many tools for KCH-based home care. Our trained telehealth nurses receive data such as weight, blood pressure, heart rate, oxygen levels, and answers to prompted questions sent electronically via a patient's home telephone line to our computer monitors for review. The patient and his or her primary care physician are alerted when any abnormal readings suggest that medication changes or treatment adjustments may be needed. The system electronically connects patients to KCH Home Health Care via a small, unobtrusive devise placed in the individual's home.
KCH Home Health Care not only bridges the care from hospital to home, but also plays a major role in the health of our communities. We offer hundreds of free blood pressure clinics, stroke assessments, as well as other special screenings and programs throughout the year in the communities we serve.
